Born on 6 May 1897 in Barrie, Ontario - son of Robert A. Stephens, Barrie, Ontario - at the time of his enlistment in 1916: present address in Barrie, Ontario; trade as clerk; single; no current military service; previously served with the 35th Regiment G.F. [Grey Foresters?] (two years); Methodist; height of 5 feet 1.5 inches; chest of 34 inches fully expanded; fair complexion; brown eyes; light hair.

Joined the 122nd Battalion, CEF, in Huntsville, Ontario, on 31 January 1916 (number 763245) - taken on the strength of the 38th Battalion, CEF, on 29 or 30 March 1918 - transferred to the Canadian Labour Pool, CEF, on 14 October 1918.
