Born on 14 March 1894 or 1895 in Monasteristhe, P. Kief, Russia - son of Mrs. Motrone Polonsky, Monasteristhe, P. Kief, Russia - at the time of his enlistment in 1917: present address in Renfrew, Ontario; trade as farmer; single; currently a member of the 38th Regiment Dufferin Rifles; Greek Catholic; height of 5 feet 7 inches; chest of 39 inches fully expanded; weight of 155 pounds; fair complexion; brown eyes; fair hair.

Joined the CEF in Niagara, Ontario, on 18 August 1915 (number 409521) - [then] - joined the 215th Battalion, CEF, in Brantford, Ontario, on 29 May 1916 - discharged[?] in December 1916 - joined the 240th Battalion, CEF, in Renfrew, Ontario, on 2 April 1917 (number 1043106) - taken on the strength of the 38th Battalion, CEF, on 11 or 12 July 1917 - wounded on 13 November 1917 - invalided to England on 19 November 1917 - transferred to the 10th Battalion, Canadian Railway Troops, CEF, on 22 May 1918.
