Born on 7 October 1898 in Etobicoke, Ontario - son of Hannah Middlebrook, Etobicoke, Ontario - at the time of his enlistment in 1915: present address in Etobicoke, Ontario; trade as farmer; single; no current or previous military service; Methodist; height of 5 feet 9.25 inches; chest of 40.5 inches fully expanded; dark complexion; blue eyes; black hair.

Joined the 126th Battalion, CEF, in Toronto, Ontario, on 24 November 1915 (number 775348) - taken on the strength of the 38th Battalion, CEF, on 4 or 6 December 1916 - wounded on 9 April 1917 - invalided to England on 16 April 1917 - hospitalized at No.16 Canadian General (Ontario) Hospital, Orrington, England - later invalided to Canada for further medical treatment.
