Born on 15 November 1889 in Poplar, England - husband of Elizabeth Johnson, Ottawa, Ontario (reported as a widower by 1918) - son of Mrs. Alice Johnson, Ottawa, Ontario - at the time of his enlistment in 1915: trade as general hand; married; currently a member of the Active Militia; previously served in the militia (three years); Church of England; height of 5 feet 5.5 inches; chest of 36 inches fully expanded; ruddy complexion; hazel eyes; black hair.

Joined the 38th Battalion, CEF, in Ottawa, Ontario, on 10 March 1915 (number 410282) - transferred to the 2nd Battalion, CEF, on 25 August 1915 - [then what?] - rejoined the Canadian Expeditionary Force in Ottawa, Ontario on 4 February 1918.
