Smith, St. Paul, Minnesota - at the time of his enlistment in 1917: present address in St. Paul, Minnesota; trade as manager or an automobile company; single; no current military service; previously served with the US Army; Congregationalist; height of 5 feet 9.5 inches; chest of 36 inches fully expanded; medium complexion; hazel eyes; light brown hair.

Joined No.3 Section Divisional Ammunition Column, CEF, in Ottawa, Ontario, on 24 March 1917 (number 2043039) - taken on the strength of the 38th Battalion, CEF, on 11 or 12 July 1917 - wounded on 5 September 1917 - invalided to England on 12 September 1917.
