Born on 25 March 1888 in Turbotville, Pennsylvania - son of David Smith, Montgomery, Pennsylvania - at the time of his enlistment in 1916: present address in Flint, Michigan; trade as carpenter; single; no current military service; previously served with the American CAC[?] (three years); Methodist; height of 5 feet 6 inches; chest of 39 inches fully expanded; fair complexion; blue eyes; brown hair.

Joined the 97th Battalion, CEF, in Windsor, Ontario, on 29 March 1916 (number 208256) - taken on the strength of the 38th Battalion, CEF, on 17 May 1917.
