Born on 28 February 1900 (gives 1898 in enlistment form) in Buffalo, New York - son of Stanley D. Lewis, Toronto, Ontario - at the time of his enlistment in 1916: present address in Belleville, Ontario; trade as mechanic; single; no current or previous military service; Church of England; height of 5 feet 7 inches; chest of 36 inches fully expanded; medium complexion; blue eyes; brown hair.

Joined the 254th Battalion, CEF, as a Lance-Corporal in Belleville, Ontario, on 16 December 1916 (number 1093156) - taken on the strength of the 38th Battalion, CEF, on 2 or 3 October 1918 - struck off the strength of the 38th Battalion to England as a "minor" on 2 December 1918.
