Born on 28 April 1894 in Ottawa, Ontario - son of Mary Emily Falls, Ottawa, Ontario - at the time of his enlistment in 1917: present address same as mother, trade as physician, Protestant, no current military service, previously served two years with the 43rd Regiment.
Joined Army Medical Corps Training Depot #4 in Montreal, Quebec, on 7 February 1917 - served with the Canadian Army Medical Corps - attached to the 38th Battalion, CEF, as a medical officer on 3 October 1918 - ceased to be attached to the 38th Battalion on 17 June 1919.
