Born on 19 February 1889 in Blackburn, Lancashire, England - son of Thomas Smith, Pawtucket, Rhode Island - at the time of his enlistment in 1917: present address in Pawtucket, Rhode Island; trade as machine inspector; single; no current or previous military service; Church of England; height of 5 feet 8.5 inches; chest of 39 inches fully expanded; fair complexion; grey eyes; light brown hair.

Joined the Signals Training Depot, Canadian Engineers, CEF, in Ottawa, Ontario, on 17 July 1917 (number 2265945) - taken on the strength of the 38th Battalion, CEF, on 2 or 3 October 1918 - transferred to the 2nd Battalion, CEF, on 7 November 1918.
