Captain Richard Benjamin O'Sullivan

Born on 16 January 1872 in Spanish Town, Jamaica, West Indies - husband of Marion Elizabeth O'Sullivan, Calgary, Alberta - at the time of his enlistment in 1915: present address same as wife; trade as medical dentist and accountant; married; currently a member of the Active Militia (no details); no previous military service; Church of England; height of 6 feet 0 inches; chest of 43 inches fully expanded; fair complexion; blue eyes; light hair.

Joined the 82nd Battalion, CEF, in Calgary, Alberta, as an honorary captain and battalion paymaster on 14 August 1915 - taken on the strength of the 38th Battalion, CEF, as a captain and battalion paymaster in England on 10 August 1916 - landed in France with the 38th Battalion on 13 August 1916 - transferred to Headquarters, 12th Canadian Infantry Brigade, CEF, on 8 July 1918 or the Canadian Army Pay Corps, CEF, on 9 July 1918 - sent to England on 5 May 1919.

Awarded the Order of the British Empire.

Awarded a Mention-in-Despatches.

Died of sickness on 1 October 1926 - buried in Beechwood Cemetery, Ottawa, Ontario.
