Born on 19 February 1895 in Russia - son of Mrs. Sabina Zuk, Kiev, Russia (Ukraine) - at the time of his enlistment in 1915: present address in Toronto, Ontario; trade as labourer; single; no current or previous military service; Greek Catholic; height of 5 feet 8.5 inches; chest of 36 inches fully expanded; fair complexion; blue eyes; auburn hair.

Joined the 126th Battalion, CEF, in Toronto, Ontario, on 14 December 1915 (number 775431) - taken on the strength of the 38th Battalion, CEF, on 4 or 6 December 1916 - struck off strength to the Canadian Forestry Corps, CEF, on 11 April 1918.
